Although costly in the beginning, dental implants often turn out to be the least expensive tooth replacement option. That's because their enviable record for longevity often outpaces dentures or bridges. Over the long-term you might spend less for implants than these other restorations.

But even with the high success rate of dental implants (greater than 95% survive the ten-year mark), we can't guarantee they won't fail. And if you're a tobacco smoker, the risk of failure might be even higher. One study, for example, found early implant failures were twice the rate for smokers over non-smokers. Although implant failures overall are low, smoking seems to be a factor in those that do.

There are a number of reasons to account for this. For one thing, inhaled smoke can damage salivary glands, reducing the flow of this important fluid. Saliva helps control bacterial growth and neutralize mouth acid, so without it you're more susceptible to tooth decay or periodontal (gum) disease. Either infection could ultimately weaken implant-supporting bone.

The nicotine in tobacco can also restrict oral blood vessels and lower their ability to supply antibodies and nutrients to the teeth and gums. This slows healing, which could have one particular effect on implant durability.

During the intervening weeks between implant placement and crown attachment, the implant's titanium post attracts bone cells that grow and adhere to its surface. The effects of nicotine on healing, especially right after implant surgery, can interfere with this integration process so that the implant doesn't anchor in the bone as well as it should.

If you're a smoker, you can increase your chances of implant success—and have a healthier mouth overall—by quitting smoking beforehand with the help of a cessation program. Or at the least, consider stopping smoking for one week before implant surgery and for two weeks afterward.

And be sure to brush and floss your teeth daily and visit your dentist regularly to keep your teeth and gums as healthy as possible. Stopping smoking and practicing effective oral hygiene could make a big difference in the success or failure of your implant.

Your risk for periodontal (gum) disease increases if you’re not brushing or flossing effectively. You can also have a higher risk if you’ve inherited thinner gum tissues from your parents. But there’s one other risk factor for gum disease that’s just as significant: if you have a smoking habit.

According to research from the U.S. Centers for Disease Control (CDC), a little more than sixty percent of smokers develop gum disease in their lifetime at double the risk of non-smokers. And it’s not just cigarettes—any form of tobacco use (including smokeless) or even e-cigarettes increases the risk for gum disease.

Smoking alters the oral environment to make it friendlier for disease-causing bacteria. Some chemicals released in tobacco can damage gum tissues, which can cause them to gradually detach from the teeth. This can lead to tooth loss, which smokers are three times more likely to experience than non-smokers.

Smoking may also hide the early signs of gum disease like red, swollen or bleeding gums. But because the nicotine in tobacco restricts the blood supply to gum tissue, the gums of a smoker with gum disease may look healthy. But it’s a camouflage, which could delay prompt treatment that could prevent further damage.

Finally because tobacco can inhibit the body’s production of antibodies to fight infection, smoking may slow the healing process after gum disease treatment.  This also means tobacco users have a higher risk of a repeat infection, something known as refractory periodontitis. This can create a cycle of treatment and re-infection that can significantly increase dental care costs.

It doesn’t have to be this way. You can substantially lower your risk of gum disease and its complications by quitting any kind of tobacco habit. As it leaves your system, your body will respond much quicker to heal itself. And quitting will definitely increase your chances of preventing gum disease in the first place.

Quitting, though, can be difficult, so it’s best not to go it alone. Talk with your doctor about ways to kick the habit; you may also benefit from the encouragement of family and friends, as well as support groups of others trying to quit too. It’s been widely established for decades that cigarette smoking contributes to cancer and heart disease. But did you know smoking will also increase your risk of tooth decay and periodontal (gum) disease, as well as nuisance problems like tooth staining, bad breath and diminished taste perception?

Its effects on your teeth and mouth are all the more reason to quit smoking. But deciding and following through are two different things: many smokers find it painfully difficult to quit due to their addiction to nicotine, tobacco’s active ingredient.

But while difficult, it can be done. Here are 4 tips to help you follow through on your decision to quit smoking.

Change Your Response to Stress. Cigarette smoking is closely tied to the pleasure and reward areas of your brain. With its “hit” of nicotine, you sub-consciously identify smoking as a way to relieve the unpleasant feelings of stress. Instead, substitute other stress relievers when it occurs: going for a walk, talking to a friend or taking a few deep breaths. In time, this substitution will wear down the trigger response to stress you’ve developed with smoking.

Gradually Reduce Nicotine. You don’t have to quit abruptly or “cold turkey”: over the course of a few weeks, try switching to brands with decreasing levels of nicotine. Each week change to a brand with 0.2-0.4 milligrams less nicotine yield than the brand you were smoking the previous week. When you reach the lowest nicotine yield you can find, begin reducing the number of cigarettes you smoke each day. Quitting Loves Company. While you’re responsible for quitting, you may also benefit from the support of others. Usually eight to ten weeks of peer group sessions, a cessation support group provides instruction and ample structure with others engaged in the same struggle. You can usually locate one of these support groups by asking your healthcare provider.

In a recent study, 92% of dental implants were found to have survived the twenty-year mark — an impressive track record for any dental restoration.

Still, implants do fail, although rarely. Of those failures, tobacco smokers experience them twice as often as non-smokers. The incidence of early failure (within the first few months after implantation) is even higher for smokers.

Early implant failure typically happens because the titanium implant and the surrounding bone fail to integrate properly. Titanium has a natural affinity with bone — the surrounding bone will attach and grow to the titanium in the weeks after surgery, forming a strong bond. An infection around the implant site, however, can inhibit this integration and result in a weaker attachment between bone and implant. This weakness increases the chance the implant will be lost once it encounters the normal biting forces in the mouth.

Smokers have a higher risk of this kind of infection because of the way tobacco smoke alters the environment of the mouth. Inhaled smoke burns the mouth’s top skin layers and creates a thick layer of skin called keratosis in its place. Smoke also damages salivary glands so that they don’t produce enough saliva to neutralize the acid from food that’s left in the mouth after eating. This creates an environment conducive to the growth of infection-causing bacteria. At the same time, the nicotine in tobacco can constrict the mouth’s blood vessels inhibiting blood flow. The body’s abilities to heal and fight infection are adversely affected by this reduced blood flow.

The best way for a smoker to reduce this early failure risk is to quit smoking altogether a few weeks before you undergo implant surgery. If you’re unable to quit, then you should stop smoking a week before your implant surgery and continue to abstain from smoking for two weeks after. It’s also important for you to maintain good brushing and flossing habits, and semi-annual dental cleanings and checkups.

Although smoking only slightly raises the chances of implant failure, the habit should be factored into your decision to undergo implant surgery. Quitting smoking, on the other hand, can improve your chances of a successful outcome with your implants — and benefit your life and health as well.

Even after decades of health warnings, approximately 45 million Americans smoke cigarettes. Although three-quarters will attempt to quit at some time in their life, most won’t be successful because smoking is both pleasurable and highly addictive.

Still, it’s in your best health interest to quit, and not just for your general health. Besides bad breath, reduced taste perception and dry mouth, smokers also face higher risk for tooth decay and periodontal (gum) disease.

Quitting is difficult because of the addictive nature of nicotine, one of tobacco’s main ingredients. Nicotine causes the brain to release dopamine, a chemical that regulates our sense of pleasure and reward. In time, this effect transcends the physical sensation — smokers soon rearrange their social, work and family life to accommodate it. For those attempting to quit, the physical and emotional effects of withdrawal are daunting.

Yet, there are a number of effective quitting strategies. Smoking is a behavior you’ve learned and reinforced over time that you must now “unlearn.” You should begin by analyzing your own particular smoking habit — when you smoke, what prompts you to smoke, what activities do you associate with smoking, etc.

Most people will find an abrupt halt to cigarette smoking all but impossible. Instead, gradually reduce the number of cigarettes you smoke each day over several weeks; a weekly 20% reduction over the previous week is a good norm. As you reduce to just a few cigarettes, you’ll be forced to choose when to smoke those “precious” few. You can also use “brand fading,” in which you switch week by week to brands with increasingly lower amounts of nicotine.

You should also attempt to replace the smoking habit with more positive habits. Keep your hands busy holding items like pencils, straws or stress balls. Snack on healthy foods, chew sugarless gum with Xylitol, and drink plenty of water. You might also join a support group of other smokers trying to quit so you don’t have to face the habit alone.

It may take several weeks to break the smoking habit. The results, though, are worth it — you may extend not only your life but the life of your teeth too.
